ACV’s Bridge to Medical Care at Home

Advent Christian Village (ACV) is a full-service community, with living options ranging from independent living to skilled nursing and even memory care; there is something for virtually everyone. ACV Home Care offers different solutions for those who need varying degrees of in-home help. But you don’t have to be an ACV member to receive these services.

Advent Christian Village Home Care Agency opened its doors to private pay patients in March of 1997 and expanded services to include those covered by Medicare in December of the same year. Since then, it has been committed to providing quality care to members of ACV, as well as those living in the surrounding communities of Suwannee and Lafayette counties. ACV Home Care delivers health care services in your home as an alternate to nursing homes, assisted living facilities, or extended hospital stays. The goal of the agency is to provide an individualized plan of care to retain or restore your independence in the comfort of your own home.

ACV Home Care has a wide variety of nursing services available. Among these are post-operative care and teaching for those who recently underwent surgical procedures to provide additional training and observation; wound care, urological care, medication management and teaching, and IV therapy, along with many other nursing services. These services often help people to avoid a long stay in a rehabilitation center or even a permanent transfer to a nursing home.

ACV Home Care also offers physical, occupational, and speech therapy. Its typical cases for physical therapy have to do with post-operative rehabilitation, gait training, and mobility and transfer training. The occupational therapy team often helps those with loss of mobility and cognitive abilities who have to re-learn in order to perform everyday tasks. Speech therapists typically assist patients by teaching problem solving techniques, judgement and reasoning skills, and working with those who are experiencing memory problems.

It also offers the services of a certified home health aide who primarily assists with personal care and activities of daily living. Home Health Aides are the heart of its newest service—the Companion Program. An aide goes into the home, not to provide personal care, but to perform more social care such as meal preparation, dog walking, completing arts and crafts projects, and even taking the person in care out into the community for various outings.
