A PHR is more in-depth than a traditional list of medications or chronic conditions. It should contain recent laboratory results, diagnostic imaging and clinical notes, in addition to any relevant information about your loved one’s care.
  3. Know what to expect at the senior living community. It’s crucial to be aware of availability and flexibility of services offered at your loved one’s new community. Is there a continuum of care available that will address their needs if anything changes? Finding out the answers to these questions will give you peace of mind and options to consider in the future.
